It’s the most terrifying feeling: a vice grip on your chest, a struggle to breathe, a cold sweat, and that unmistakable tingling down your left arm. These are classic, critical symptoms of a heart attack, and they demand an immediate call to the emergency room or ambulance. Don't wait.
But what if your heart is perfectly fine, and the culprit is something simpler—like the spicy meal you just devoured?
In my practice, I often see patients who initially mistook a bad case of acid reflux for a cardiac event. It’s an easy mistake to make, and one we should all be aware of.
Acid reflux is surprisingly good at mimicking serious heart issues. The primary reason is simple anatomy: your oesophagus sits right behind your heart, smack-dab in the middle of your chest.
When stomach acid and gastric juices flow backward into the oesophagus (heartburn), the resulting pain can feel identical to heart-related chest tightness.
Furthermore, acid reflux can cause breathing difficulties. If acidic fluid regurgitates into your throat and is accidentally inhaled into your lungs, it triggers a choking or gasping sensation, which can understandably cause significant fear and anxiety.
While symptoms can overlap, the context often provides the biggest clue.
| Heart Attack | Acid Reflux (Heartburn) | |
|---|---|---|
| Timing | Usually occurs during or shortly after physical exertion. | Tends to occur when relaxed, often after eating or drinking (especially oily or spicy foods). |
| Relief | Unlikely to be relieved by antacids. | Often relieved quickly by over-the-counter antacids. |
| Position | May worsen with exertion, but not typically position-dependent. | May worsen when lying down or bending over. |
| Other Symptoms | Often accompanied by nausea, lightheadedness, or pain radiating to the jaw, neck, or back. | May include a sour taste in the mouth or regurgitation of food/fluid. |
Sometimes acid reflux can even occur on an empty stomach if you’ve missed a meal and acid has built up.
While distinguishing between the two is helpful, these tips are for informational purposes only. I cannot stress this enough: if the symptoms are sudden, severe, and you have any suspicion it might be a heart attack, seek emergency medical care immediately.
It is always prudent to rule out a cardiac emergency first. Medical professionals prioritize stabilizing vital parameters to ensure your heart is healthy. Once a heart attack has been definitively ruled out, we can take our time to investigate and manage underlying digestive issues.
Don’t gamble with your health. If you experience alarming chest symptoms, play it safe and head to the emergency room.
In addition to food choices, small changes to your daily habits can significantly improve reflux symptoms.
If you are experiencing ongoing, frequent heartburn or acid reflux symptoms, schedule an appointment with an acid reflux specialist to discuss long-term management and treatment options.
